  • Overview

    Under the supervision of the Manager, Behavioral Health Care Coordinator will provide care management to members with mental health needs. This role is primarily telephonic and allows the care coordinator to provide the support needed for ongoing care coordination needs to enhance quality, improve member outcomes, and encourage appropriate utilization of behavioral health services.

    Responsibilities

    • Provides care management of members who have mental health needs to include members identified for the Enhanced Care Coordination (ECC) initiative with community behavioral health partners.
    • Coordinate services for individuals with behavioral health, substance use and/or developmental needs.
    • Conducts ongoing screening of the member for changes in medical and behavioral health status.
    • Identify gaps or barriers preventing the member from following through with a recommended treatment plan.
    • Provides information on network access and identifies and implements interventions and resources that address and close gaps in care.
    • Facilitates communication and coordination between members of the health care
    • team to include the member, family or caregiver, the PCP/provider, targeted case managers with the local Community Services Boards, other health care and/or CMHRS providers, and the community to maximize health care responses, quality and cost-effective outcomes
    • Educates the member, the family or caregiver, and members of the health care team about treatment options, community resources, insurance benefits, psychosocial concerns, care management, etc., so that timely and informed decisions can be made.
    • Assists the member and their family/caregiver in navigating the behavioral health care system
    • Encourages the member to be actively involved in the health care decision-making process
    • Empowers the member to problem solve by exploring options of care, when available, and alternative plans, when necessary, to achieve desired outcomes
    • Encourages the appropriate use of behavioral health care services and strives to improve the quality of care and maintain cost effectiveness on a case–by-case basis.
    • Performs secondary review of CMHRS service authorization requests.
    • Collaborates with UR nurse to manage length of stay based upon medical necessity, community supports, and member’s specific social situation.
    • Provides referrals for brief and immediate crisis intervention (i.e. Adult and Child Protective Services).
    • Collaborates with the transition coordinator to ensure member’s needs are met when undergoing transitions
    • Provides referrals to health-related services such as disease management, social work and health education
    • Provides assistance to members in crisis
    • Strives to promote member self-advocacy and self-determination
    • Acts as an advocate for a member’s health care needs
    • Participates in VPHP’s ongoing quality improvement process
    • Participates in continuing educational activities as appropriate
    • Participates in care management rounds/meetings
    • Practices in accordance with applicable local, state and federal laws which govern confidentiality and medical information privacy regulations (HIPAA)
    • Other duties as assigned

    Qualifications

    MINIMUM EDUCATION REQUIREMENTS

    • Preferred Licensed Mental Health Professional in Virginia, or;
    • Preferred Master’s Degree in Health or Human Services, or;
    • Bachelor’s Degree in Health or Human Services, or;
    • Licensed Practical Nurse in the Commonwealth of Virginia, or;
    • Board of Counseling registered Qualified Mental Health Professional
    • Minimum of 3 years’ experience working with individuals with mental health, substance use and/or developmental disability 

    SPECIAL KNOWLEDGE AND/OR SKILLS

    • Excellent Patient care evaluation skills and the ability to communicate well (written and verbal) with all levels of management, medical staff and employees.
    • PC literate with a strong working knowledge of Microsoft Office and various computer software programs
    • Working knowledge of resource options, and experience in making appropriate referrals
    • Strong decision making skills, ability to handle multiple priorities
    • Knowledge of case management fundamentals
    • Knowledge of community mental health rehabilitative services, developmental disability waivers, and addiction recovery treatment services.

    WORK BACKGROUND/EXPERIENCE

    • Previous work experience with behavioral health populations, low income and disadvantage populations
    • Experience in care management, assessment, patient care management or community-based resource delivery
    • Experience in the recovery model of care
    • Experience working with Community Service Boards, State Psychiatric Hospitals, Freestanding Psychiatric Hospitals, and/or Developmental Disability training facilities
    • Knowledge of community resources required
    • Demonstrated ability to work effectively in multi-disciplinary teams 

    PHYSICAL REQUIREMENTS

    • Physical health sufficient to meet the ergonomic standards and demands of the position
