    Overview

    Our Social Workers are responsible for the timely coordination of quality health care services to meet the member’s specific health care and social needs in a cost-effective manner to promote positive outcomes. Performs investigations and holistic assessments, provides intervention support or referrals and works effectively in a multidisciplinary team. 

     

    Medical Management educates members and empowers them to become active participants in their coordinated health care plans. This team of knowledgeable professionals in their field provide services to include Utilization Review, Medical Outreach, Case Management, Disease Management, Credentialing, Social Work as well as Grievance and Appeals.

    Responsibilities

    • Collaborates closely with the care management team to ensure social determinants of health have no negative impact on the member
    • Undertakes casework for more complex cases in an elderly/disabled population.
    • Assesses members through phone calls and home visits and assists with development of the individual care plan (ICP) to meet their needs.
    • Coordinates community resources for provision of basic services.
    • Refers members for screening of LTSS and other waiver services
    • Provides referrals for brief and immediate crisis intervention (i.e. Adult Protective Services).
    • Conducts complex investigations and holistic assessments to include the member’s physical, mental and psychosocial state.
    • Provides appropriate service referrals to members and support to their families and caregivers.
    • Acts as liaison with the local department of social services and other community agencies
    • Assists members with maintaining their Medicaid eligibility

    Qualifications

    MINIMUM EDUCATION REQUIREMENTS

    • BSW required
    • MSW preferred  

    SPECIAL KNOWLEDGE AND/OR SKILLS 

    • Strong oral and written communication skills
    • PC literate with working knowledge of Microsoft Office and various other computer software programs
    • Ability to work and communicate with diverse cultural groups
    • Familiarity with medical and elder care resources
    • Demonstrated ability to work effectively in multi-disciplinary teams

    WORK BACKGROUND/EXPERIENCE 

    • Minimum of 3 years clinical experience and/or current managed care experience
    • Experience working with elderly and/or disabled populations
    • Experience working in community based long term care

    PHYSICAL REQUIREMENTS

    • Physical health sufficient to meet the ergonomic standards and demands of the position.
    • Some local travel may be required
